UMFC has a vacancy for a full-time Practitioner TCP based in Wodonga or Wangaratta to work therapeutically with designated TCP client(s).

What is a TCP? A Targeted Care Package:

  • Is for those children and young people who are at risk of going to a residential care
  • Provides an alternative to a residential care placement and enables better outcomes
  • Is an allocation of funding and target that is tailored specifically to meet the individual needs of a particular child or young person
  • Is based on an assessment of that child or young person's needs
  • Provides funds that are utilised for one or more clearly articulated supports to meet the child's needs

The TCP Practitioner will support children / young people and their carers and/or families. The objectives of the position are:

  • To provide high-quality support to children / young people who are supported by Targeted Care Packages
  • To achieve better outcomes for children and young people in out of home care or upon reunification with family by acting as part of a creative, flexible suite of supports to children / young people, volunteer caregivers and families
  • To actively and collaboratively participate in a care team approach
  • To participate in the development of high-quality support strategies for clients and ensure adherence to the relevant DHHS Registration Standards and UMFC Policies and Procedures

The successful applicant will have the knowledge and demonstrable experience in delivering innovative and effective approaches for young people with complex needs. Knowledge of the Family Services and Child Protection statutory requirements as well as demonstrated ability to achieve positive outcomes for young people through a strengths-based, outcomes-focused case management approach, will be required in this role.

Relevant tertiary qualifications in Youth Work, Social Work, Community Services or equivalent and experience in the field will be vital for this role.
