About the role

We are on the lookout for dedicated and passionate team members with experience working with people with disability and/or those in the aged care sector. You will hold a case load of both NDIS and Aged Care package recipients and provide support to build capacity to maintain independence for as long as possible. You will have the opportunity to work collaboratively with other experienced health professionals to maintain and build upon the capabilities of your clientele and you will have the rewarding opportunity to assist with working with individuals to identify and meet their goals in this consumer directed care model.

We are looking for

Sound experience of the knowledge and skills required in working with frail older people, people with disabilities and their carers.
Well-developed interpersonal and communication skills, including the ability to initiate and maintain contacts with a broad range of service providers and community agencies.
A sensitive approach to working with individuals and a philosophy of empowerment.
Knowledge of the Case Management approach to service delivery, in a rural community setting.
Ability to work as part of a multi-disciplinary team.

Selection Criteria
Essential : Commitment to the GV Health Values of Compassion, Respect, Excellence, Accountability, Teamwork and Ethical Behaviour and ability to exhibit behaviour which reflects our values.
Essential : A Tertiary qualification in an appropriate Health and/or Human Services field, or extensive, relevant experience.
Essential : Sound experience of the knowledge and skills required in working with frail older people, people with disabilities and their carers.
Essential : Well-developed interpersonal and communication skills, including the ability to initiate and maintain contacts with a broad range of service providers and community agencies.
Essential : A sensitive approach to working with individuals and a philosophy of empowerment.
Desirable : Sound case management skills, with a focus on capacity building, planning and financial monitoring skills.
Desirable : Demonstrated ability to negotiate access to appropriate services.
Desirable : An understanding of the brokerage model of service delivery.
Essential : A satisfactory National Criminal History Record Check, Victorian 'Employee' Working with Children Check and completion of a Commonwealth of Australia Statutory Declaration will be required from prospective candidates prior to commencement of employment.
